Summary of Performance Expectations: The Clinical Care Manager (CCM) coordinates patient care working synergistically with the inter-professional care team, the patient and family to achieve quality outcomes through efficient care.
Provides evidence based culturally sensitive care, monitors patient and healthcare outcomes and applies best practice nursing strategies.
Provides coaching to direct care staff related to clinical performance and measured outcomes.
Participates in resolving unit issues or patient complaints.
Leads and coordinates patient flow utilizing all resources both internal and external.
Oversees patient experience using evidence-based practice.
Provides input in the development and revision of policies appropriate to clinical practice and patient population.
Designs and implements patient care strategies for disease specific populations as needed.
Education and Credentials: Required: Graduate of accredited BSN nursing program.
Current NC RN license or licensure from Nursing Compact State.
BCLS certification.
If the BCLS certification is not from the American Heart Association (AHA), an AHA certification is required within three months of employment date.
Master of Science in Nursing Degree Requirements : Effective May 1, 2016 newly employed and current employees promoted into the following positions are required to possess a MSN degree or a Master’s degree in a related field.
Clinical Care Manager Clinical Development Coordinator Nurse Director o Patient Care Manager Professional Development Coordinator Resource Coordinator/Professional Development Coordinator The MSN degree or Master’s degree in a related field must be completed within five years from the date of employment or placement into the position.
Additional Requirements by Practice Area Emergency Department: ACLS and PALS and/or ENPC upon employment (PALS and/or ENPC must be maintained bi-annually).
Non-Violent Crisis Intervention Certification within six months of employment.
Psychiatry Services: Non-Violent Crisis Intervention Certification within three months of employment.
Birthing Center: ACLS and NRP within three months of employment date.
Newborn Nursery and Level III Nursery – NRP and PALS within three months of employment date.
STABLE within one year of employment date.
STABLE renewal not required.
Pediatrics: NRP and PALS within three months of employment date.
PACU: ACLS and PALS within one year of employment.
Day Surgery: ACLS and PALS within one year of employment.
Preferred: Master’s degree in Clinical Nurse Leader; Clinical Nurse Leader certification; National nursing certification in specialty area of practice.
Work Experience: Required: Effective July 1, 2017, newly employed and current employees transferring into this role are required to possess five or more years of nursing experience along with experience as charge nurse, preceptor, team lead, successfully leading projects, or BCLS, ACLS, PALS instructor.
Demonstrates leadership potential.
Given training and on-the-job experience incumbent should be proficient in the basic aspects of the job within three months of employment date.
Preferred: Experience in coordination of clinical care.
