*Job Description* *Position: Clinical Program Director (LCSW or LCAS)* *Reports To: Operations Director* *Summary:* As a member of the Clinical Team, the LCSW/LCAS will provide support and mental health services to patient population as requested or initiated by provider/patient.
LCSW/LCAS will provide community resources and coordinate referrals as indicated.
Primary Duties & Responsibilities: 1.
Provide on demand integrated care consults to a diverse patient population in a fast-paced setting in collaboration with care team.
Population can include pediatrics, adolescents, families, adults with IDD/MD/SUD diagnoses.
Services include screenings, assessments, support, brief interventions, and referrals as deemed appropriate.
* Provide psychoeducational information as appropriate for patients and/or family members or accompanying caregivers * Provide traditional counseling services and support to patients and family members.
* Develop and maintain familiarity with related psychopharmacology * Collaborate alongside behavioral health team to ensure holistic health care and support of medical team.
* Provide training to staff as needed * Remain current and knowledgeable about local and regional resources for mental health and substance abuse treatment, developing collaborative relationships where possible * Participate in outreach educational activities in the community as needed * Maintain a regular schedule at assigned location in collaboration with treatment team * Participate in quality improvement projects as assigned * Participate in all meetings and trainings as assigned * Complete Comprehensive Clinical Assessments as needed Compliance and documentation *Thoroughly, accurately, and promptly document all service rendered in the electronic medical record for every patient.

*Computer Skills:* To perform this job successfully, an individual should have knowledge of Microsoft Word, Microsoft Excel, QuickBooks, payroll systems, and internet software.
*Education/Experience:* * Master's degree (M.

) or equivalent and required trainings per State Board * At least 2 years of experience working with Mental Health, Intellectual and Developmental Disabilities, or Substance Use populations.
*Certificates and Licenses:* * Valid North Carolina driver's license * Master’s Degree in Social Work, Human Services, or equivalent * Current LCSW and/or LCAS North Carolina licensure * Current FA/CPR Compliance * Experience in integrated care setting preferred *Knowledge, Skills, and Other Abilities:* * Time management * Oral and written communications skills * Professionalism * Reliable transportation * Critical thinking * Ability to analyze and interpret from various sources to identify trends *Physical Demands:* The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
