Apply Now Brief Job Summary This position performs professional social work to individuals and/or families with personal, social, health and economic needs.
Work in this class involves providing intensive social work services in serious and complicated cases which require the flexible use of a wide range of social work skills and intervention techniques.
Work may involve investigation of alleged neglect or abuse; providing in-depth assessment of family dynamics and needs, assessment, intervention and treatment of patients and/or families with acute to severe medical and/or emotional disorders and mental illnesses; serving as expert witness at court hearings; or advance social work intervention, and instruction.
Work is distinguished from the Social Worker II classification by greater involvement in sensitive and traumatic situations such as custody and removal of client from homes, catastrophic and terminal illness, and/or family dysfunction.
Employees supervise and train lower level social workers, students, and interns.
Employees typically report to a Social Work Supervisor II under general supervision.
Job Responsibilities • Performs case intake duties as assigned; receives reports of abuse, neglect, or exploitation and prepares related reports for supervisor screening; completes the North Carolina intake report.
• Maintains intake logs; researches clients to determine if they receive public assistance; searches multiple databases.
• Processes requests for assistance from other counties; enters information into electronic log; prepares paperwork.
• Assigns case numbers; assists clients in the completion of applications for a variety of programs and services; completes assessments and reassessments to determine client needs.
• Prepares oral and written referrals to district attorney or law enforcement agency as appropriate.
• Manages a caseload of clients; assists clients in making medical appointments, obtaining transportation, etc.
; attends appointments with clients as needed; completes FNS, Medicaid, and other paperwork for clients; maintains case files and updates information; completes client referrals.
• Manages clients in the Employment and Training program as assigned; monitors compliance with program rules and regulations; assists in coordinating training and employment opportunities for clients.
• Manages a caseload of Special Assistance In Home Program clients as assigned; makes home visitor monitor client needs and goals; completes required home assessment reports; assists problems in identifying and resoling problems; utilizes home visit data to complete quarterly review and reassessment documentation; completes payment tracking for clients; assists clients with payee services as needed.
• Provides routine monitoring of Adult Care Home, Family Care Home and Adult Care facilities in the county to ensure compliance with state rules and regulations to protect client rights, health, and safety.
Investigates complaints received regarding facilities and completes corrective actions as needed maintains related documentation.
• Manages a caseload of guardianship wards as assigned; substitute decision maker to act on behalf of the ward, to make decisions involving the management of personal affairs, property or both.
Conducts home and facility visits to monitors status of wards; performs initial and ongoing assessments to evaluate client needs; creates and implements service plans; completes Guardianship Status Reports; evaluates guardianship cases.
• Manages a caseload of Community Alternative Program for Disabled Adult clients; as assigned performs client assessments and reassessments; conducts home and facility visits; requests nursing assessments from nursing agencies; creates prescription requests for medical doctor; coordinates services with other agencies; maintains case documentation.
• Performs quality control duties as assigned; reviews agency records for compliance with agency, local, state, and federal guidelines; gathers data and prepares reports; evaluates current strategies for meeting mandated goals and requirements and prepares suggestions to update strategies; remains informed of changes to relevant guidelines; recommends employee training.
• Performs case planning duties; completes case files and records; prepares comprehensive court reports in order to review the case and develop action plans; attends case planning review meetings.
• Files petitions with the court for court intervention as needed; develops, prepares, and revises court reports; attends and testifies at court hearings.
• Completes initial and annual required training.
• Maintains detailed case documentation and completes a variety of regular and special reports; maintains confidential records and files.
• Performs related duties.
Minimum Qualifications The case management personnel shall meet one of the following qualifications: a.
Bachelor’s degree in social work from an accredited school of social work, and one (1) year of directly related experience of community experience (preferably case management) in the health or medical field directly related to homecare, long-term care, or personal care, and the completion of a DMA-certified training program within 90 calendar days of employment.
b.
Bachelor’s degree in a human services or equivalent field from an accredited college or university with two (2) or more years of community experience (preferably case management) in the health or medical field directly related to homecare, long-term care, or personal care and the completion of a DMA certified training program within 90 calendar days of employment; c.
Bachelor’s degree in a non-human services field from an accredited college or university with two (2) or more years of community experience (preferably case management) in the health or medical field directly related to homecare, long-term, or personal care and the completion of a DMA certified training program within 90 calendar days of employment.
d.
Registered nurse who holds a current North Carolina license, two (2) year or four (4) year degree, one (1) year case management in homecare, long-term care, personal care or related work experience and the completion of a DMA certified training program within 90 calendar days of employment.
