Position Summary:
The nurse navigator provides individualized assistance to tumor site-specific patients and their families in overcoming barriers to care, facilitating timely access to care, and providing education and support.
The cancer patient’s trajectory through diagnosis, treatment, and aftercare survivorship will be seamless and coordinated in collaboration with the multidisciplinary team.
Essential Functions
Collaborates with the multidisciplinary team to coordinate patient care.
Educates patients and their caregivers on the disease process, treatment, side effect profile, and survivorship.
Coordinates referrals both with Frye and within the region.
Provides emotional support related to the clinical condition.
Advocates for patients and caregivers for appropriate utilization of resources.
Implements and measures quality improvement initiatives
Communicates consistently with providers ensuring a seamless plan of care as well as follow-up care.
Minimum Qualifications:
Minimum Education
Associate's Degree in Nursing Required
Bachelor's Degree in Nursing Preferred
Licenses/Certifications
Basic Life Support (BLS)
Licenses: Current RN license valid in NC
